  TEXT  MESSAGE REMINDERS!

We are currently using a service called MJOG which will automatically send out a text message the day or two before your booked appointment, to remind you of the the time and date of your appointment

We may also use this service to send out other personal messages,  for instance, if the doctor wishes you to make an appointment to discuss results of recent blood tests.    

In order for this service to work, we need to have your mobile telephone number in your electronic medical record.    If you think we do not already have this information, please do let us know your number. 

If you do not wish to receive text messages on your mobile phone, you can ask to be excluded from this service.    Please let us know if this is the case

How to Register
Non-English Speakers

New Registrations

registrationWe have an open list and warmly welcome requests for registration from patients currently living or moving into the area.

All new patients who wish to register with the practice will need to complete our Registration forms, these are avaliable by:-

  1. Calling into reception at the surgery
  2. Or you can now print a copies of the registration forms below, ALL the forms below need completing.
  1. Registration Form
  2. New Patient Health Questionnaire
  3. Alcohol Screening

Please note all *3* forms need to be completed.

New Babies & Children under 5

Parents of new babies are advised to register the child as quickly as possible. You can print a copy of the registration form below.

All registrations forms must be returned to the Surgery in person. Please bring a form of ID with you showing your address (this does not have to be your current address if you have recently moved) e.g. driving license or utility bill.

We offer an recommend a New Patient Check appointment to all patients over the age of 5. This is a valuable check-up as it can identify potential problems and introduces you to the practice and to the services we can offer you and your family. Please being a urine sample with you.
